To pre-register please download the following forms, fill them out and bring them in person to the surgery signed with the necessary documentation (see below).

The ideal times to come to St Marks Medical Centre are between 11.00 - 14.00 as these are quieter hours, for Nightingale Surgery please see opening times. Coming to St Marks outside these hours may result in you waiting as clinics will be running, evenings are particularly busy especially Friday nights. If you need to come in the evening due to commitments we would recommend you come on Mondays between 18.30 and 20.30.

To register with the GP you will need proof of your identity and proof of your current address to determine that you are in the practice catchment area. Call the practice if you require anymore information or if you are not able to provide these and we will be able to help you.

Registration appointments are most often booked a week after you hand in the registration forms. (If you require an appointment straight away, please speak to reception regarding your situation and they will advise).
We will then make an appointment to see the Practice Nurse for a new patient health check. This will make sure that our records are accurate and all your routine health checks are up to date. Any health information held by your previous GP will also be sent to us as your new practice but this can take up to six weeks to arrive.
Registration appointments are with the nurse, she will do a general check-up B/P, height, weight and a Urine sample will be needed as well. Patients will not be officially registered until they have attended the nurse appointment. Any cancelled appointments that are not made within the next week after the initial appointment may result is the de-registration of the patient.
If you are on any repeat medication please bring a list of your prescribed drugs to show to the Nurse when you attend for your new registration check.
Each GP practice has an area map to show which patients it takes on.
If you live within the blue area you can register with the practice.
You can still register if you live outside our catchment area, but this would be an out of area registration which means that the practice do not contractually have to provide home visits. Please speak to one of the reception staff for more details.
If you are currently registered with the practice and you move outside the practice catchment area you may continue to be registered if you move to a new home. Your continued registration is at the discretion of the practice and will depend on an assessment of the clinical risk of living not within the immediate catchment area. Visit www.nhs.uk to find out the list of practices in your new area.
If you are ill while away from home or if you are not registered with a Doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.
You can be registered as a temporary patient for up to three months. This will allow you to be on the local Practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

Please note it is up to the discretion of the practice to register you as a temporary patient. You will need to come into the practice with the necessary documentation to sign the forms before you will be registered with the practice for temporary services
